Registered Nurse - Delray AreaActiva Home Health (an AccordCare Company) is a licensed Medicare certified home health agency seeking an experienced full-time Field RNs to cover South to Central Palm Beach County. POSITION SUMMARY

Total Compensation Package - $75,000 - $82,000 (depending upon experience)

Ask about our $3,000 sign-on Bonus

The RN will work to provide the highest quality of care to patients utilizing the Plan of Care provided to ensure patient's needs are met in relation to safety, well-being and satisfaction. Bilingual Spanish speaking is a HUGE plus but not required. WellSky/Kinnser & Home Health experience is a plus.

JOB RESPONSIBILITIES

* Provides safe and effective care for the client based on individualized Plan of Care.
* Provides prompt communication of changes in patient medical status/emergencies to Team resulting in effective management of cases.
* Provides safe and effective nursing for the patient, based on the nursing scope of practice and individualized Plan of Care in conjunction with Nurse Supervisor/Manager of Clinical Practice. May include, but not limited to, medication administration, wound care, infusion, transferring & positioning patient, catheter care, effective communication with the client, family, and Team Case Manager
* Completes Starts of Care, using OASIS Assessment tool and performs supervisory visits.
* Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.

REQUIRED EXPERIENCE & EDUCATION

* Current RN license, state of Florida
* Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
* Must have wound care and infusion skills
* 1 year of Home Health Medicare experience preferred
