Job DescriptionDuties & Responsibilities

  • Supervise and support CHWs, Health Promoters (HPs), and Health Educators (HEs), including support for training coordination, development and implementation, and related projects.
  • Provides support for Caridad Center CHWs, HPs, & HEs to facilitate internal and external workgroups related to CHW training and other workforce issues, relevant workgroups, and other chronic disease-related activities.
  • Supports Caridad Center in responding to inquiries for information and maintaining the website.
  • Assists with CHWs, HPs, & HEs independent contracts supervision.
  • Collects and compiles community health worker-related resources.
  • Maintain accountability for all CHWs, HPs, and HEs entering all educational workshops and classes' demographics and outcome information for reporting purposes.
  • Responsible for maintaining the data and record of CHW's working and volunteer hours and the total number of workshops, classes, and demos performed monthly. Report to the department director or data team by the 5th of every month.
  • Make phone calls to CHWs for educational activities and training.
  • Set order and distribution of CHWs uniforms.
  • Phone call for Confirmation of recruitment for CHWs for training and activities.
  • Home visit records data entry.
  • Collect, update and maintain CHWs Contract. (Home visit or educational activities with required documents).
  • Update the Health Education Programs Coordinator of any changes for Educational Activities in the PET Calendar.
  • Scan and maintain records of all the educational activities.
  • Coordinates and oversees purchases, contracts, reports, mailings, and travel arrangements for all related CHWs, HPs, and HEs activities.
  • Maintain an active participation with the Florida CHWs Coalition
  • Support CHWs requests by phone and electronically from the Florida Board Certification regarding CHWs.
  • Reviews reports and other documents for completeness, accuracy, and content.
  • Participate in on-site and off-site training, continuing education programs, and in-service training to meet CHW certification, funding, or regulatory requirements and meet the community's needs.
  • In collaboration with the department director, develop and coordinate a core competency model for CHPs & HEs.
  • Contributes and is an active part of a positive and professional team, supporting the organization's mission.
  • Functions within flexible work schedule (including but not limited to days, early evenings, and weekends) as assigned by site and partnership manager and as required by the job.
  • Manages and maintains educational, visual, and incentive inventory for workshops.
  • Submit monthly and quarterly reports on the 5th of every month.
  • Attend meetings and training as required.
  • Work in support of and cooperation with CHC staff.
  • Other duties as assigned. Programmatic Responsibilities
    • Understand of, and commit to health equity, including racial equity and addressing the social determinants of health.
    • Collaborate with the recruitment effectively, interviewing, hiring, and termination of CHWS, related staff, and volunteers.
    • Aides in the development of monthly reports provided by the outreach team.
    • Must be able to work independently.
    • Ability to receive and offer feedback constructively.
    • Ability to handle complex and stressful situations with professional composure
    • Provide department presentations to partnership organizations, patients, and community members. Qualifications
      • Active Florida Board CHW certification
      • Post-secondary education, with at least five years of experience in a public health environment, specifically outreach coordination.
      • Must demonstrate strong interpersonal, oral, and written communication skills.
      • Bilingual (English & Spanish) proficiency, computer literate, effective collaboration, and culturally competent.
      • Must be computer literate and have effective collaboration, communication, and documentation skills.
